Calling all 1st Year to 5th Year students of Tarlac State University!
Application is FREE of charge. Submission does not guarantee approval, as applicants are prioritized based on available funds and slots.
Requirements:
1. Personal Information Sheet (PIS) - provided by the office
2. Photocopy of Certificate of Registration (COR) – 2nd Semester 2025-2026, signed by the University Registrar
3. Photocopy of Report of Grades (ROG) - 1st Semester 2025-2026; must have no failing or incomplete grades, regular load
4. Photocopy of School ID
5. Certificate of Indigency
6. 2x2 ID Pictures
Category Requirements (if applicable)
1. Photocopy of PWD ID - for Persons with Disability
2. Photocopy of Solo Parent ID - for Solo Parents or their Dependents
3. Photocopy of Certificate of Confirmation (COC) - for Indigenous People (IP)
REMINDERS:
• Incomplete applications will NOT be processed - make sure all documents are complete, valid, updated, and legible
• Applicants currently receiving another scholarship are NOT eligible
• New applicants will be placed on a waiting list until additional slots become available
